Purpose

To inspire creative ways to:

  • create awareness in doctors, dentists and nurses,
  • reduce misdiagnoses,
  • help patients connect with others,
  • put your ideas to work!

These grants will enables passionate people in our community to dream up innovative ideas for projects that make a difference. The IPPF wants to support and encourage our committed members to make a difference in their community!

The IPPF is giving away ONE $500 and TWO $200 grants to support an inspiring project in our community plus each Project Leader will receive free Registration Fees for the 2010 IPPF Annual Meeting!*

Selected entrants are to make an impact in their community by coordinating a project with the $500 they receive from the Foundation.

* Actual amount of grants awarded will depend on the number of complete and qualified packages received.

Eligibility

  • Entrants must be at least 12 years old. Entrants under 18 years of age will require permission from parent or legal guardian to participate.
  • Deadline to submit project ideas. Projects must be actually received by the IPPF offices by midnight on Friday, February 19, 2010 to be eligible to be awarded a grant.
  • Maximum of three applications can be submitted per person.
  • Groups are able to submit applications, but will need to designate primary contact "project leader" for the project.
  • Project leaders will be selected and notified.

Proposal

The project should be achievable, innovative, and have a positive impact that touches the lives of people in a significant way. It must include:

  1. Description of Project (3 page max.): Include information on target audience; outline of outreach process; timeline; statement of goals and how progress will be measured, reported and evaluated;
  2. Budget (1 page): Include a budget of expected costs to complete proposed project. Costs to complete full project may exceed the $500 grant but will not be compensated by the IPPF.

Project leaders will be selected based on the perceived impact of the submitted idea to our community and liklihood of the entrant's ability to effectively carry out the plan.

Procedures

  1. Once all entries are received, a review committee will narrow the list of project ideas. Winning projects will be selected based on review committee decision.
  2. Acceptable applications include ideas that are not copyrighted. Entrant takes responsibility that all information submitted is not copyrighted material.
  3. Applications not containing the required information, applications in excess of the stated limit, and applications not meeting the application criteria will be disqualified. Employees, trustees and officers of IPPF and the immediate family members and household members of each are not eligible. Becoming a grantee is subject to validation and verification of eligibility and compliance with these Rules, Terms and Conditions. Failure to comply will result in disqualification and selection of an alternate grantee. If any project is deemed ineligible, it will be disqualified and the runner-up applicant will be declared the potential grantee.
  4. Project leaders will be contacted by phone and/or email provided during registration.
  5. Project leaders and the title of the entry will be announced on the website. The projects will be featured on IPPF website.
  6. IPPF will be in contact with project leaders by March 1, 2010
  7. Return of any grant or grant notification as undeliverable will result in a disqualification and an alternate winner may be selected.
  8. No transfer or substitution of any award will be permitted, except at IPPF's sole discretion.

Use/Rights

Entrants agree to grant the IPPF license to use the project idea associated with the entry. Each entrant agrees and grants IPPF permission to make available and display and otherwise distribute project idea in all media in perpetuity without compensation. All project ideas submitted become property of IPPF.

  1. Selected project ideas, will be posted on various websites as IPPF see fit.
  2. IPPF is not liable to an entrant for acts or omissions arising out of that entrant's participation.
  3. Entrants participating in this challenge are liable for any copyright or trademark infringements caused by their project.
  4. Entrant is responsible to secure permission from everyone included in their project. IPPF assumes no responsibility for royalties or fees associated with others who may be included in the project.
  5. All entries must be submitted by February 19, 2010. IPPF assumes no responsibility for lost or late entries.
  6. IPPF reserves the right to reject entries that are considered inappropriate for the challenge.
  7. IPPF reserves the right
    • to suspend or cancel the challenge in their sole discretion if a computer virus, bug or other technical problem corrupts the administration or security of the challenge,
    • to select winners from among all eligible entries received prior to the cancellation date, and
    • to conduct background checks on selected project leaders.
  8. IPPF reserves the right to modify the rules of the challenge in any way or at any time, as long as reasonable notice is given.
  9. The rules of this challenge may change if situations beyond IPPF's control occur between now and the conclusion of this challenge. Any changes to the rules will be posted on the webpage.
  10. By participating in this challenge, entrants agree to be bound by these rules and by the decisions of IPPF, which are final in all matters relating to this challenge.
  11. Grants are nontransferable.
  12. Arrangements for the fulfillment of all grants will be made by IPPF. Receipt of any grant is conditioned upon compliance with any and all applicable federal, state and local laws, rules and regulations.

Each entrant agrees to indemnify IPPF from any third party claims arising from or related to that entrant's participation. By participating, entrants agree that IPPF and those involved in the development, production, implementation and distribution of this promotion and their respective parent companies, affiliates, subsidiaries, service providers, and agencies, and their directors, officers, agents, employees, attorneys, and any other person or entity associated with such entities and/or promotion shall not be liable for any and all claims, damages, losses or injuries, including any third party claims, arising from or relating to, in whole or in part, this challenge, including entry and participation in this challenge and acceptance, possession, use or misuse of the grants.

All entrants further agree that in the event of any third party claims, damages, losses or injuries, arising from or relating to this challenge, they will indemnify IPPF from any and all such claims, including the attorneys' fees related thereto. IPPF shall not be responsible for any technical malfunctions of the telephone network, computer on-line system, computer equipment, software, program malfunctions or other failures, delayed computer transactions or network connections that are human, mechanical or technical in nature, or any combination thereof including any injury or damage to entrant's or any other person's computer related to or resulting from downloading as part of this challenge.

Any person attempting to defraud or in any way tamper with this challenge will be ineligible for grants and may be prosecuted to the full extent of the law.

Distribution of Funds

  1. Selected project leaders will be granted $500 for funding their project idea. Family members, immediate or extended, do not qualify to receive payment for services rendered or time provided through this challenge.
  2. Project Leaders agree, in exchange for any funds distributed, to complete the project proposal Final Report with Measurements of Goals and Statement of Accomplishment as defined in the project proposal.
  3. IPPF and Grantee will complete final agreement and distribution, include delivery of report of goals measured and achieved and settlement of all project expenses to the grant limit of $500.
  4. Each project leader will be reimbursed for actual project expenses (with receipts) on a monthly basis to limit of grant.

OWNERSHIP OF APPLICATION MATERIALS: By applying, each applicant, and collectively, the Team, unconditionally assign and transfer to IPPF all rights, title, and interest in and to the application materials, including, without limitation, the copyright therein. IPPF shall have the right, in its sole discretion, to duplicate or alter the application materials for any purpose which IPPF deems necessary or desirable. In the event that it is subsequently determined that IPPF does not own the application materials, each Contestant grants to IPPF, its successors, licensees and assigns the irrevocable, fully paid-up, perpetual, and exclusive license to use the application materials in any manner at any time or place, or for any purpose which IPPF deems necessary or desirable, and in any present or future media. Each applicant agrees that IPPF may, without any limitation or further compensation, use his or her name, voice and/or likeness in any and all media for the purpose of advertising and promoting the Project Awards and IPPF. Neither applicants nor grantees shall be entitled to receive any compensation or credit for use of application materials, other than that disclosed in these Rules, Terms and Conditions.

Each applicant is responsible for ensuring that he or she has the right to apply and submit his or her materials in accordance with these Rules, Terms and Conditions.

Awards are governed by the laws of the State of California, with venue and jurisdiction in California, and all claims must be resolved in the courts of California. By submitting an application, each applicant agrees: (1) to release IPPF from all liability, loss or damage arising out of their participation in the Service Awards and with respect to their acceptance and use/misuse of any grants; (2) to be bound by these Rules, Terms and Conditions; (3) that the decisions of the judges are final and binding on all matters relating to the Project Awards; and (4) where legal, to the use of their names, likenesses and application materials for advertising and promotional purposes in all media worldwide (including online) without additional compensation, except in Tennessee and where otherwise prohibited by law.

Winner is responsible for all federal, state and local taxes. An application, except where prohibited by law, constitutes a grant of permission to IPPF winners’ names on its website and in connection with a Winners’ List without further notice or without additional compensation.

IPPF is not responsible for any incorrect or inaccurate information, whether caused by website users or by any equipment or programming associated with or utilized in the Grants, or by any technical or human error that may occur in the processing of applications, including but not limited to any misprints or typographical errors. IPPF assumes no responsibility for any error, omission, interruption, deletion, defect, delay, theft, or destruction of, or unauthorized access to, or alteration of, applications.

IPPF reserves the right to modify, suspend or terminate the Grants for any reason that compromises the legitimacy of the Grants or otherwise affects IPPF’s ability to fairly administer the Grants.
