Pemphigus and Pemphigoid Health Management Program Launched
Sacramento, CA -July 1, 2008 – The International Pemphigus & Pemphigoid Foundation (IPPF) and Centric Health Resources (CHR) have partnered to create the Pemphigus and Pemphigoid Health Management Program.
The pemphigus and pemphigoid patient community now has a new support resource for patients, and a systemized way to collect and measure outcomes and measure quality of life improvements. Through the use of targeted and timely assessments and educational interventions, delivered via phone by a trained Peer Health Coach, the program aims to positively impact outcomes such as reducing the risk of steroid use, managing stress, and decreasing the number of adverse drug events, hospital visits, and missed work days. The program utilizes Centric’s proprietary health management program development process and software tools, complete with branching logic capabilities and customizable data collection and reporting features.
What is Health Management?
The IPPF and Centric believe that patients with chronic illnesses require more than just clinical therapy. Their treatment should be comprehensive, addressing every aspect of the patient’s life (including medical, work, relationships, family, stress, and financial). By focusing on the patient, positive outcomes will result for everyone: patients, payers, manufacturers, and physicians. The IPPF and CHR accomplishe this through an innovative health management approach.
The GOAL of a health management program is to optimize the effectiveness of the patient-physician relationship through a comprehensive system of educational and communication INTERVENTIONS including:
- educational materials
- health measures
- peer health coach support
At the core of the program is CHR’s unique utilization of a Peer Health Coach (PHC). A PHC is a patient with the particular disease who is extensively trained to contact participants on a regular basis and deliver assessments (survey questions) that collect data specifically related to measure intended outcomes. The PHC delivers real-time non-medical interventions. Interventions include but are not limited to education, counseling, and enhancing the communication between the patient and physician and care team.
These interventions are designed to drive BEHAVIOR CHANGE; that is, more and better decisions by the patient related to their health, such as nutrition, protocol adherence, regular health monitoring, recognizing and dealing with stress, and the importance of addressing health issues with their physician and care team.
The RESULT of positive behavior change benefits all constituents involved in the patient’s care:
